SAP Certification Information

Dear Student Assistance Program Applicant:

Thank you for your interest in becoming a Student Assistance Program (SAP) Counselor. As you are aware, you must be apprenticed before you can begin this process and meet the following requirements. Please be sure to send a copy of your current apprentice or certified substance abuse certificate along with this application.

1. Experience: 2000 hours as an SAP Counselor

2. Supervision: 70 documented clinically supervised hours (see supervisor's sheet attached, which must be filled out)

3. Education: 100 education hours in 8 performance domains (see attached form)

4. References: three references: clinical supervisor, building/school supervisor, and community representative (see attached three forms)

Below are links to the application forms for you to fill out. Please return them to the above address, all together, with complete documentation for your training, as soon as possible with the appropriate fees. The total cost for this process is $125.00, which must accompany your application.

If you fail to send adequate documentation, all the completed forms, or the appropriate payment, your application processing will be delayed. Please note that all certification fees are nonrefundable, so be sure you meet all of the criteria necessary to apply for this credential before sending in your application or fees.
